Medical Community Guidelines for Treatment
The American Society Of Addiction Medicine (ASAM) recently published this statement as a matter of public policy. “Abstinence, usually accepted as the primary goal of treatment, is not feasible as an exclusive goal for all opioid dependent persons. Buprenorphine maintenance is effective and safe and is an integral part of addiction medicine. Ideally, buprenorphine treatment includes behavioral, psycho-dynamic, and 12-step approaches combined with pharmacological interventions to provide a broad spectrum treatment for opioid-dependent patients.”

Alcohol, because it is legal, is by far the most currently abused substance, in both the United States and the world. Alcohol comes in four types, with ethyl alcohol (grain alcohol, or ETOH) as the type humans ordinarily consume. It is often taken socially to lessen inhibitations and provide a sense of relaxation. In a non-social setting, alcohol is typically taken to escape stress and provide relaxation.

In 1994, the use of oral naltrexone(ReVia ) was approved for the treatment of alcoholism. Naltrexone (ReVia) is a narcotic antagonist that blocks the pleasurable effects of alcohol and reduces cravings.
In 2006, the use of intra-muscular injection naltrexone (Vivitrol) was approved by the FDA.
“Craving” is defined as a powerfully strong desire and perceived need for an experience. Neurochemical alterations caused by chronic exposure to addictive agents form the biological basis of drug/alcohol cravings.
ReVia is the brand name of oral naltrexone hydrochloride manufactured by DuPont. Vivitrol is the brand name of intra-muscular injection form of naltrexone manufactured by Cephalon Pharmaceuticals.
Naltrexone is an opioid antagonist which completely reversibly blocks subjective effects of intravenous opioids for 24 - 72 hours. The mechanism of action is unknown, though it is thought to reduce cravings. Naltrexone does not cause an Antabuse(disulfiram)-like reaction. Placebo-controlled trials have been shown to double abstention rates of alcohol.
Naltrexone is a non-addictive and safe medication which uses pharmacologic means to improve the likelihood of successful treatment for alcohol dependence. Acknowledging that alcohol dependence is really a medical disease with powerful physiological components points to an objective use of a medication to aid in the treatment of this disease. While this medication may not be for every patient suffering from alcohol dependence, it will aid the treatment program in offering yet another means to complement ongoing support and treatment.
